Thank you for your willingness to help the students of the Middletown Unified School District! We recognize and value the volunteers who provide a variety of services to our schools. Volunteers are viewed as role models to students,  and individuals who can provide support throughout MUSD’s programs. This webpage contains information and outlines procedures to ensure that all volunteers have been screened appropriately to provide for maximum safety for all students in MUSD.

 Parents, students, and community members who fulfill the mandatory volunteer requirements established by the Middletown Unified School District may volunteer.  In order to determine the requirements appropriate for the level of service you wish to provide, the following definitions and requirements are explained below. 

All volunteers in any capacity must have all of the prerequisites completed and on file with the Human Resources Office.  Parents and/or guardians are not prohibited from being active in their children(s) education; however, they may be limited in volunteer capacities due to prior convictions and/or probationary periods.
 

STEPS TO BECOME A CLEARED VOLUNTEER 
 

  1. Complete a Volunteer Data Form . This form is to be completed every school year.
  2. Submit a valid Tuberculosis Test (TB test). Submit every 4 years.
  3. Valid clear fingerprints. Use the Live Scan Document to take to any facility that does fingerprinting – Only if this is your first time volunteering in our district. 

PLEASE NOTE: If you are using our local Middletown Mail and More. DOJ & FBI -$47 and Rolling Fee $25. 

Once all documents and fingerprinting have been received you will receive and email from Human Resources to begin volunteering.

FINGERPRINTS CAN TAKE UP TO 30 DAYS TO RECEIVE RESULTS.

 

CONTINUE BELOW IF YOU ARE WANTING CLEARANCE TO BE A DRIVER FOR A SCHOOL RELATED ACTIVITY OR SPORTING EVENT. 

DRIVING & TRANSPORTING STUDENTS
Individuals transporting students in a personal or district vehicle must register YEARLY with the District:

Complete Employee and Volunteer District Clearance Driver Form. This form is for all employees and volunteers needing clearance to drive a school vehicle.

Complete Personal Vehicle Registration Form. This form is clearance to drive your personal vehicle for a school sponsored trip or athletic event. 

Complete Authorization to Running Driving Record. This form is a clearance to run your driving record. 

If you need to drive within a 48 hour window, please submit a copy of your driving history. 

Please have the following documents readily available when completing the form online. You will be asked to upload all documents.

  • Copy of valid California Driver's License
  • Copy of valid Vehicle Insurance of Personal Vehicle
    • Please note that you must have the legal California insurance limits required by law.

This form must be completed every school year in order to be considered eligible to drive students in a personal and/or district vehicle.

Information to consider:

  1. If you are asked to drive a district vehicle you are covered under the MUSD vehicle insurance.
  2. If you are driving your own vehicle your vehicle insurance is the primary.

When driving your own vehicle:

All student passengers shall submit permission slips signed by their parents/guardians. Teachers shall ensure that each driver has a copy of the permission slip for each student riding in his/her vehicle.

The number of passengers, including the driver, shall not exceed the capacity for which the vehicle was designed. Trucks may not transport more persons than can safely sit in the passenger compartment. The driver shall ensure that the manufacturer's recommendations for his/her vehicle are followed regarding the seating of children in seats equipped with airbags.

The driver or any other person shall not smoke or have in his/her immediate possession a lighted pipe, cigar, or cigarette containing tobacco or any other plant when there is a minor in the motor vehicle, whether the motor vehicle is in motion or at rest.  (Health and Safety Code 118948)
